As problem of title, I have received the problem report from quality assurance department.

I would like to inform about problem report detail.

• Problem detail
  Product : AOTG18LVC(E004424) *which was manufactured at 26^th Aug 2010
  Problem contents :
  The bridge rectifier was short. And, the heat paste had been drying.
  the product was operate correctly at 3 years.
• Analysis result
  There was similar problem at Japan also.
  Detail information about its problem is below.
  *Product ; AO-A250, AO-A220, AO-A360, AO-A22W, AO-J22W
  *Number of problem ; 5 cases
  *Cause;
  )1 At IPM installation process in factory, there were 2 process of temporary tightening and final tightening.
  Both operation was carried out at one factory worker(same worker).
  Therefore, there is possibility that factory worker forgot the final tightening process.
  
  )2 The motor screw driver which had been used at IPM installation process was used at another screw clamp process.
  Therefore, there is possibility that there is variation as torque of motor screw driver.
  
  )3 There is possibility that screw clamp was done to slanted due to grease application of IPM.
  
  *Measure
  )1 Work process was divided at temporary tightening and final tightening(24^th march 2011).
  
  )2 Motor screw driver became to dedicated one for screw clamp process of IPM.
  Then, attached the cover for not able to change the torque level by factory worker.
  
  And, factory become to do a periodic inspection as motor screw driver(Appendix 1).
  
  )3 Made the work instruction document for check as to slanted screw clamp and screw clamp missing(from 7^th Apr 2011)(Appendix 2).
• Speculation of this result’s cause.
  FGL guess it is same problem of Japan’s problem.
• Measure
  same as Japan’s one.
• Measure to product
  Already installed product; ; measures by each location if problem is occur.
  Stock product; no measures.
  Manufacture product ; already measured.
